Assignment Details 

Job Description:

  • Patient Assessment    
     1.    Conduct Intake Screenings at West Hills and in emergency rooms and other facilities as requested on individuals experiencing crisis and are presenting as requiring more thorough intervention than the usual available.
     2.    Complete Intake Screenings with potential patients for admission into hospital programs, utilizing admissions criteria.
     3.    Provide Intake Screening of incoming patient’s mental health and chemical dependency through various methods.
     4.    Prepare written reports of Intake Screening results and prepares recommendations for the level of care and treatment needs.
  • Treatment Planning
     1.    Evaluate patient needs and advocate the lowest level of care consistent with the defined patient needs.
     2.    Interface with the facility staff that provides the appropriate level of care, to ensure a smooth transition for the patient.
     3.    Promote an effective use of resources for patients, customers and the facility with sensitivity to cost of health care.
     4.    Observe patient to detect behavioral patterns and report observations to medical staff.
     5.    Assist with completing initial admissions forms for patients as required.
     6.    Intervene to restrain suicidal, violent, or potentially violent persons by appropriate means.
     7.    Begin patient/family education at time of admission or provide information to family during intervention to increase their understanding and ability to cope. 
     8.    Provide information about the patient to the program staff who will be receiving the patient.
     9.    Ensure proper tracking of all reviews.
  • Specific Standards for the Care of Child & Adolescent Patients Ages 5 through 17
    1.    Maintain knowledge of the Child/Youth Services Program rules, protocols and handbooks guidelines.
    2.    Maintain knowledge of children’s stages of development and seek guidance from the nurse in charge, M.D. or supervisor when questions about the norms for behavior related to child development arise.
    3.      Screen patients for behaviors consistent with the developmental stages and make note of any deviations in behavior.
    4.    Recognize acting out behavior of child specific age group and set appropriate limits in clear, concise, behavioral and non-judgmental terms based on the age of the patient.
    5.    Demonstrate an awareness of learning disabilities and translates treatment/program expectations to the learning disabled child.
    6.    Relate to child patients in an age appropriate manner.
  • Specific Standards for the Care of Adult Patients Ages 18 through 64
    1.    Maintain knowledge of the developmental tasks and sociological theories of this age group.
    2.      Gather information from the patient, family or other interdisciplinary team and screen based on knowledge of adult behaviors.
    3.      Utilize theoretical concepts to assess adults within Specific Standards of Care for this age group.
    4.      Maintain knowledge of mental health and chemical dependency treatments, medications and side effects of such in this age group.
    5.     Maintain current knowledge of the Adult admission criteria.
    6.     Recognize behaviors and set appropriate limits in clear, concise, behaviors and non-judgmental terms.
    7.     Maintain knowledge of the Adult Program and Chemical Dependency Program rules, protocols and handbooks guidelines.
    8.     Seek guidance from the department supervisor, nurse in charge, or M.D. when questions arise related to the screening of these individuals.
  • Specific Standards for the Care of Geriatric Patients Ages 65 and older
    1.    Maintain knowledge of the developmental tasks and sociological theories of aging.
    2.      Gather information from the patient, family or other interdisciplinary team and screen based on knowledge of geriatric adult behaviors.
    3.      Utilize theoretical concepts to assess adults within Specific Standards of Care for this age group.
    4.      Maintain knowledge of mental health and chemical dependency treatments, medications and side effects of such in the geriatric age group.
    5.     Maintain current knowledge of the Adult admission criteria.
    6.     Recognize behaviors and set appropriate limits in clear, concise, behaviors and non-judgmental terms.
    7.     Maintain knowledge of the Adult Program and Chemical Dependency Program rules, protocols and handbooks guidelines.
    8.     Seek guidance from the department supervisor, nurse in charge, or M.D. when questions arise related to the screening of these individuals.
  • Admission Process
    1.    Maintain knowledge of the admission process and be able to complete all tasks required, including all documentation accurately and timely.  Ability/knowledge will be evaluated within the first 90 days of employment.
  • Additional Standards
    1.    Adhere to facility, department, corporate, personnel and standard policies and procedures.
    2.    Attend all mandatory facility in-services and staff development activities as scheduled.
    3.    Adhere to facility standards concerning conduct, dress, attendance and punctuality.
    4.    Support facility-wide quality/performance improvement goals and objectives.
    5.    Maintain confidentiality of facility employees and patient information

Qualifications

  • R.N. or Master’s-prepared mental health related field required and  LCSW, LMFT, LSW, MFT preferred.  Crisis Prevention Intervention (C.P.I. given by facility but must be able to pass) and CPR - BLS
